What information is needed to create a lab order and why is it required?

The information needed to create a lab order is:

First Name

Last Name

Phone Number

Email Address

Street Address

City

State

Zip Code

Date of Birth

Gender

Your address is needed to obtain a licensed physician in your state and to send correspondence if there are abnormal or critical results. Same with your telephone number, if there is an abnormal or critical result, direct access to you is imperative. Your email address is required to match results within our systems. Gender and date of birth are required as some results have variable ranges based on gender and age. We are also unable to provide services to those under the age of 18, therefore, the date of birth is needed for verification.
